GUIDELINES FOR OFFICE BASED PATIENT SELECTION & SCHEDULING

Note: These guidelines do not apply to the other forms of sedation we offer (such as nitrous oxide).

An anesthesiologist is available to address questions regarding scheduling, testing and patient selection for procedures in office based settings. In order to identify patients with appropriate co‐morbid risk profiles for  procedures in this setting and address questions regarding patients with specific co‐morbidities for  anesthesiologist* review the following criteria are defined:  

I. PATIENTS NOT APPROVED FOR PROCEDURES REQUIRING OFFICE BASED ANESTHESIOLOGY SERVICES

  •  Respiratory
    • BMI >30 **
    • Obstructive Sleep Apnea
    • Documented history of difficult laryngoscopy/intubation
    • Home oxygen requirement
  •  Cardiovascular
    • Mechanical Valve replacement
    • CHF with EF < 35%
    • CIED (ICD, loop recorder, Bi‐Ventricular pacemaker, pacemaker)
    • Unstable angina
  • Renal
    • Dialysis
  • Pregnancy
  • Diabetes Mellitus
  • Neurologic / Psychiatric
    • Frequent or poorly controlled seizures
  •  Hematologic
    • Hgb < 8; Platelet count < 90
  • Metabolic
    • Known history of malignant hyperthermia
    • Known history of prolonged responses to muscle relaxants
  • ASA III

– Respiratory

  • BMI >30 **
  • Obstructive Sleep Apnea
  • Documented history of difficult laryngoscopy/intubation
  • Home oxygen requirement

– Cardiovascular

  • Mechanical Valve replacement
  • CHF with EF < 35%
  • CIED (ICD, loop recorder, Bi‐Ventricular pacemaker, pacemaker)
  • Unstable angina

– Renal

  • Dialysis

-Pregnancy

-Diabetes Mellitus

-Neurologic / Psychiatric

  • Frequent or poorly controlled seizures

– Hematologic

  • Hgb < 8; Platelet count < 90

-Metabolic

  • Known history of malignant hyperthermia 
  • Known history of prolonged responses to muscle relaxants

-ASA III

II. PATIENTS REQUIRING ANESTHESIOLOGIST REVIEWPRIOR TO DAY OF PROCEDURE

  1.  Respiratory  
    • OSA risk  
    • Head/Neck cancer with Rad Tx or persistent airway impact  
    • Poorly controlled asthma‐ ER visit in last month or frequent use of rescue inhalers
    • Pneumonia or URI within last 3 weeks 
  2. Cardiovascular  
    • Valvular Heart Disease  
    • Congestive Heart Failure  
    • Coronary Artery Disease  
      • MI or PCI within prior 6 months  
      • PCI: Stents requiring maintenance of perioperative antiplatelet agents  
  3. Renal  
    • Creatinine > 2 gm/dl  
  4. Hematologic  
    • Anticoagulants  
  5. Miscellaneous  
    • Age > 85  
    • Neurologic  
    • History of stroke (CVA) with residual deficits  
    • Non‐ambulatory patients (e.g. wheel chair bound)  
    • Behavioral or Cognitive (dementia) issues that preclude independent living 
    • Substance abuse/tolerance  
    • Previous problems with anesthesia
